Please scan your forms and save as a PDF, JPEG, TIFF or PNG file. For passports the scan must be clear, in color, and include the full picture and signature pages. Once you have scanned then saved the files to your computer fill out the fields and click the file upload button. You can attach up to 4 files using this form.
